Working Together with the new SEN Code of Practice
Dr Sarah Temple and the EHCAP team offer bespoke personalised training packages for Education, Health and Care Teams who are working with Education Health Care (EHC) Plans.

Person Centred Practice (PCP) will be key to the success of EHC Plans in terms of transforming how we all work and improving outcomes for children and young people with additional needs into adulthood. Local Authority (0-25) SEN Teams are working together with education, health and care professionals to provide effective Person Centred assessments. Some counties have adapted the Common Assessment Framework into a Person Centred Early Help Plan which will feed into EHC Plans - others are using tools such as the One Page Profile.
